How To Schedule and Prepare For Your Appointment.

To schedule your appointment, please call our office at tel 253-268-0720

Please arrive to your appointment 15 minutes earlier to check in and fill out the new patient packets if you haven’t filled it out online.

Obtain your medical records, ask the primary care provider to send chart notes, X-rays and any lab results regarding the patient’s condition to our fax number 253-466-7072.

Medications and Refills

Please contact your pharmacy for medication refills, medications are refilled at your next appointment. Please keep those appointments to ensure your medication is refilled on time without interruption.
